Project Power for adults is an innovative type 2 diabetes lifestyle

change program from the American Diabetes Association® that

raises diabetes awareness and offers diabetes risk reduction

education to people with diabetes and prediabetes nationwide.

By promoting healthy lifestyles, healthy eating, and increased

physical activity, this 12-week diabetes risk reduction program will

educate the public on diabetes and remove barriers to treatment—including

destigmatizing diabetes and identifying and removing social determinants

of health-related barriers. Over the duration of the program, health care

provider coaches work closely with participants in a small group setting

to help them break old habits, create new ones, set attainable goals, and

keep them motivated.

Project Power participants will access highly engaging digital tools such as

the Project Power app to monitor their progress and help them stay on track.

The app includes an easy-to-use food diary, weight and activity tracker,

connectivity, secure discussion forum, short animated educational videos,

health care provider coach/participant communication, and automated alerts

and notifications.

Program participants will receive a wireless scale, activity tracker, as well as

other program supplies and incentives.

goalsetting, and personalized nudges for positive behavioral change to develop healthier habits that

lead to improved health outcomes. Participants have the support of a small group of members just like

you, sharing the same challenges and working towards achieving similar goals. The program includes

education for skill development, strategies to enhance motivation toward goals, and identification of
